It depends on exposure far more than on presence. Most crawl spaces have some growth, and that on its own is not an emergency. What matters is how much, whether crawl space air reaches your living space, and who is in the house.

Almost every page that ranks for this question is written by someone who sells remediation. Here is the baseline they tend to leave out.
Mould spores are present in essentially all indoor and outdoor air. EPA states plainly that there is no practical way to eliminate all mould and mould spores indoors, and that the way to control indoor growth is to control moisture. A crawl space, by design, places wood a short distance above soil in a space that is usually neither heated nor cooled — so finding some growth there is a common finding, not a discovery that something has gone badly wrong.
That reframing matters commercially as well as practically. “You have mould” is true in most houses and is not, by itself, a reason to spend thousands of dollars. “Your crawl space is wet enough for mould to spread across the framing” is a different statement, and it is the one worth acting on.
What the health agencies say
Summarised from public guidance rather than from anyone selling a service. This is general information, not a diagnosis.
EPA reports that exposure to mould can cause allergic reactions such as sneezing, runny nose, red eyes and skin rash, can irritate the eyes, skin, nose, throat and lungs, and can trigger asthma attacks in people who are allergic to it. CDC similarly describes upper respiratory symptoms in otherwise healthy people and notes greater risk of more serious effects for people with weakened immune systems or chronic lung disease.
Two honest caveats worth carrying. First, individual sensitivity varies enormously, so two households in identical crawl-space conditions can have completely different experiences. Second, the popular idea of a uniquely dangerous “toxic black mould” is not well supported by the public health literature, and colour does not identify a species in any case. The recommended response to significant indoor growth does not change with the type: remove it and fix the moisture.

Widespread growth with a musty smell reaching upstairs is worth acting on; a few patches in a dry crawl space usually is not an emergency.
Most have some growth somewhere, and that is the honest baseline nobody selling remediation has much reason to tell you. Mould spores are present in essentially all indoor and outdoor air, and a crawl space puts wood a short distance above damp soil in an unconditioned space. EPA is explicit that there is no practical way to eliminate all mould and spores indoors; the way to control it is to control moisture. So the useful question is not whether you have any, but whether conditions are letting it spread.
Crawl space air does reach the living space. Warm air rising through a house draws replacement air in from the lowest point, so odours and airborne particles from a crawl space can end up indoors, and leaky ductwork running through the space accelerates it. Estimates of how much indoor air originates below the floor vary widely and are often quoted with more confidence than the evidence supports. What is reliable: if you can smell the crawl space upstairs, air is moving from there to here, and that is worth fixing regardless of what is growing.
You cannot identify a species by looking at it. Many common moulds appear black, green, grey or white, and the colour tells you nothing reliable about which one it is or how it will affect you. CDC has noted that the popular idea of a uniquely dangerous 'toxic black mould' is not well supported, and that the sensible response to any significant indoor growth is the same: remove it and fix the moisture. If identification genuinely matters, for a health question or a dispute, that takes sampling by an independent assessor rather than a photograph.
There is no legal or health-based limit, and no agency publishes an 'acceptable' figure, which is precisely why the question is hard to answer honestly. The practical line most people use is EPA's cleanup guidance: growth under roughly 10 square feet is generally homeowner scale, and larger areas warrant a contractor with containment. Treat that as a scale-of-work threshold, not a safety threshold. The more meaningful measure is whether the crawl space is dry, because dry crawl spaces do not grow mould regardless of what is already there.
Usually not first. EPA's position is that sampling is generally unnecessary if visible growth is present, because the response does not change: remove it and fix the moisture. Testing earns its cost in narrower cases, such as a health question your doctor wants information for, a real estate dispute, or verifying that a large remediation actually worked. If you do test, use an assessor who does not also sell the remediation.
Find out why that area is wet, before spending anything on the growth itself. Check grading and downspouts outside, look for standing water or damp soil, check whether the vapour barrier covers the ground, and put a hygrometer down there for a day. Growth is a symptom with a moisture cause, and cleaning without correcting the cause reliably brings it back.
